Thats unfortunate that they did not send you one. I received one in the mail around Tuesday dec 6th. However, I had alread downloaded one and sent it in shortly before I received the one in the mail. They were suppose to all be put in the mail by Dec. 2.
Talked briefly to one of the lawyers on the phone today, and he said the 3rd bulk transfer should be complete in 2 weeks at the outside. I am still holding my breath.
